Diabetes Prevention Program

GemCare Wellness is approved by the Centers for Disease Control and Prevention (CDC) as a “Diabetes Prevention Program.” Our team of Registered Dietitians delivers a virtual proven program that emphasizes the need to make lasting lifestyle changes through a supportive virtual setting over the course of one year to reduce the risk of developing Type 2 diabetes.
Using the CDC-supported curriculum, coaches educate and facilitate interactive discussions among groups whose goal is to lose 5-7% of their starting weight by the end of the year-long program. Classes cover a variety of health and wellness topics including meal planning, tips for eating away from home, and ways to increase daily physical activity. Sixteen group classes are delivered in the first 6 months of the program, and 6 group classes are delivered in the last 6 months of the program.
OUR PROGRAM PROVIDES:
HEALTH COACHING
Through a virtual classroom setting, this program takes place over the course of 1 year including 16 weekly courses and 6 monthly courses using approved curriculum from the CDC.
ESTABLISHED GOALS
Together, we establish goals for healthy eating and increased physical activity. The program is designed to help you make positive, long-lasting lifestyle changes.

American adults has prediabetes?
You may have prediabetes and be at risk for Type 2 diabetes if you:
- Are 45 years of age or older
- Are overweight
- Have a family history of Type 2 diabetes
- Are physically active fewer than 3 times per week
- Were diagnosed with diabetes while pregnant (gestational diabetes) or gave birth to a baby that weighed more than 9 pounds
Do you QUALIFY?
- 18 years or older with BMI > 24kg
- No previous diagnosis of Type 1 or 2 Diabetes
- Have been clinically diagnosed with Prediabetes or gestational diabetes during a previous pregnancy
